DCCHS aims to provide primary and allied health care that promotes physical and mental wellbeing by optimising the health status within individual's own environment. The Discovery Coast Community Health Service offers the following free of charge services by appointment to the communities of Agnes Water, Baffle Creek, Bororen, Lowmead, Miriam Vale, Rosedale, Turkey Beach and surrounding communities:

Health Assessments

Health Assessments are a one-off holistic approach to health care which comprises physical checks of blood glucose, blood pressure, foot assessment, balance, grip strength and BMI (Body Mass Index). Anyone who is found to have concerns will be referred to an appropriate health professional.

Preventative Health

The DCCHS aims to prevent and reduce the rates of obesity, diabetes and heart disease in our community. We provide and maintain access to relevant health promotion and preventative health programs and activities designed to promote health and wellbeing by taking a holistic approach to preventative health in the community with a range of activities. Heartmoves Program

Heart Foundation Heartmoves program is a gentle physical activity program suitable for anyone who hasn't done any exercise in a while. You can exercise at your own pace in a friendly atmosphere. 

Heartmoves is run by accredited exercise professionals specifically trained in managing safe, low to moderate intensity physical activity programs. 

Heart Foundation Heartmoves is open to everyone and is designed to be safe for people with stable long term health conditions such as heart disease, diabetes or obesity. Heartmoves is also suitable for anyone who hasn't done any exercise in a while.

Active Ambassadors

Active Ambassadors play a vital role in the delivery of activities of these programs.  If you are interested in becoming an Active Ambassador in your local community phone 4902 1100 to register.

The Youth Worker assists and supports youth today for healthier living tomorrow.

This youth friendly health service recognises that young people have specific health problems, needs and concerns and adolescence is a stage of life in which behaviours are formed and many health concerns begin. Appropriate and positive health care services can improve outcomes for young people and prevent long-term difficulties. The programs' focus is on prevention rather than intervention and is primarily for young people aged 12 to 24 years.The Youth Worker is able to assist individual young people and / or their families / carers with choices to improve their health, regarding a wide range of issues including:

 


Develop understanding;
Resilience;
Self-management;
Inter-personal relationship skills;
Harm minimization; and Safe practice.
Young people have the opportunity to participate in age appropriate community based activities in and around the Discovery Coast to enable them to develop social skills, relax and have fun in a supportive recreational setting. Services delivered are provided with a minimal charge or are free of charge, are non-judgmental and confidentiality is assured. DCCHS receives funding from the federal Department of Health and Ageing under the Regional Health Service program.
